Nursing Genogram Template
A three-generation genogram template with health history markers for family nursing assessments, BSN and MSN coursework, and clinical documentation.
What This Template Shows
Three generations with health markers across multiple family members.
Both paternal grandparents are deceased; the grandfather with heart disease, the grandmother with diabetes.
The maternal grandfather carries a hypertension marker.
The father carries both hypertension and diabetes markers. The mother carries an anxiety marker.
The hereditary pattern is visible before any clinical analysis is added: cardiovascular and metabolic risk running on the paternal side across three generations, anxiety on the maternal side.
The primary person (the daughter) sits at the base of that pattern. A close relationship with the maternal grandmother and distance between the parents complete the relational layer.
This is the kind of diagram a nursing student would produce for a family or community health assessment and be expected to analyze for care implications.
The symbol legend is displayed below the diagram. For full definitions and clinical usage, see Genogram Symbols Explained.
When to Use This Template
- Community health nursing family assessments: document household composition, hereditary health patterns, and family relationships as part of a family-centered care assessment.
- BSN and MSN coursework and clinical assignments: meets standard three-generation requirements for family and community health nursing courses aligned with NLN competencies.
- Family nurse practitioner intake: map family health history and hereditary risk during initial intake to inform care planning and chronic disease management.
- Chronic disease management: identify which family members share risk factors for conditions like diabetes, hypertension, and cardiovascular disease across generations.

FAQ
What is a nursing genogram template?
A nursing genogram template is a three-generation family diagram used in nursing practice and education to map household composition, hereditary health conditions, and family relationships. It gives nurses and nursing students a visual record of the family's health history and relational structure; the basis for family-centered care assessment, health risk identification, and care planning.
How is a nursing genogram different from a medical genogram?
Both use the same standard notation and health markers, but the clinical focus differs. A medical genogram is used by physicians and genetic counselors primarily to track diagnoses, hereditary conditions, and disease patterns for clinical decision-making. A nursing genogram takes a broader view; it includes household composition, relational dynamics, and the family's capacity for support and self-care, alongside the health history. The nursing context is holistic and care-planning focused.
What health markers does a nursing genogram include?
EasyGenogram uses a four-quadrant system on each person symbol. Common nursing-relevant markers include heart disease, diabetes, hypertension, cancer, stroke, anxiety, depression, and substance use. Each quadrant is assigned a condition category and filled in when the condition is present. The assignment is noted in the legend below the diagram.
Is this template free for nursing students?
Yes. Open and customize it in EasyGenogram at no cost. Students with a valid school email can export free when a professor registers a classroom account; other users export from $9/mo/seat.
Can I use this for a BSN or MSN community health assignment?
Yes. The template follows McGoldrick-Gerson-Petry standard notation and includes the health markers required for family and community health nursing coursework. Open it in EasyGenogram, adjust the family structure and health history to match your assignment, and export as PDF for submission. Students with a valid school email can export free.
