Therapist Exposure Recording Form

The Therapist Exposure Recording Form is a practitioner-only in-session OCD EX/RP record. It helps clinicians document the exposure target, response-prevention expectations, repeated SUDS ratings, comments, difficulties, and post-exposure learning during a clinician-guided exposure.

Use this background section to understand when the tool fits, how practitioners use it, and what to consider before sharing it with clients.

Practitioner-Only Operation

The practitioner controls the record during the session. The client contributes subjective SUDS and behavioral participation but does not manage the scoring or note interface.

Trajectory Boundaries

The form can show start, end, minimum, maximum, and interval SUDS descriptively where ratings support it, but it does not interpret habituation, learning strength, or the next clinical step.

Is there a client form for this tool?

No. The primary product experience is practitioner-only during in-session ERP.

Can SUDS be entered manually?

Yes. Manual entry is supported when fixed intervals are not practical during a live exposure.

What happens if an interval is missed?

A missed interval appears as a gap rather than being interpolated.

Does the form decide the next ERP step?

No. It records the exposure and learning; the practitioner decides what to adjust next.
