ERP Practice Record

The ERP Practice Record is an OCD ERP practice-session log for practitioners whose clients are completing exposures between sessions or during structured homework. It helps record which exposures occurred, peak anxiety or SUDS, urge to ritualise, end anxiety, whether the ritual was prevented, and what the client learned.

Learn more about this tool

Use this background section to understand when the tool fits, how practitioners use it, and what to consider before sharing it with clients.

When It Is Used

Use it after ERP practice attempts so the next review can address what happened during exposure and response prevention.

What It Captures

Each entry captures exposure description, peak anxiety/SUDS, urge to ritualise, end anxiety, ritual-prevention status, and learning notes.

Important Limits

It does not create homework assignments, interpret learning quality, label practice as success or failure, or prescribe next steps.

Is the entry completed before or after exposure?

After exposure. Each entry represents a completed ERP practice attempt.

Is ritual-prevention status required?

It is required by default because it is central to ERP review.

Can the record be used without a prefilled assignment?

Yes. Assignment context can be linked to a specific exposure or left as client-described practice.

Does the tool evaluate whether the client’s learning is correct?

No. It records the learning note for practitioner review without judging it.
